MASCOUTIN SOCIETY OF CHICAGOLAND TRAINING LIBRARY ORDER FORM Training Media maintained by the Mascoutin Society of Chicagoland in the form of audio tapes or video tapes are kept by the Librarian for free use by members whose dues are current. Requests for use of training media must be made by individual members by filling out an order form. Training media will be delivered only to individual members. They are thereby able to exercise responsibility for their use and timely return. The Mascoutin Society of Chicagoland and the borrowers will use hand delivery at monthly society meetings or other society gatherings to minimize transportation time required for the training media. Borrowers agree to return the training media to the Librarian within 30 days of the date that they were delivered to them (at the following month society meeting). Borrowers may request as many training media as they wish on one order form; however, only a maximum of four (4) training media will be delivered to one borrower at one time. This will insure wider distribution of the training media and shorter waiting lists. Orders for training media will be filled in sequence as the orders are received. In the event that a training media is not available, the Librarian will advise the requesting member of the approximate available date. This will be calculated on the basis of 30 days for each order ahead of him on the waiting list. Please advise the Librarian if you wish to be removed from the waiting list. Only training media marked with an asterisk (*) may be copied. All others are copyrighted and licensing prohibits copying.
